(Reference Code: KAM) Provide independent integrated case management to New Hampshire seniors and adults with chronic illnesses and/or disabilities in accordance with agency policy, applicable state regulations and accepted professional standards of practice. Work is performed with minimal supervision and a high degree of self initiative and independence in keeping with the standards of excellence set forth by the program. Qualifications: *Current RN license issued by the State of NH Board of Nursing *Working knowledge of behavioral health, substance abuse and human service resources. *Experience working as a team member with the demonstrated ability to exercise good judgment, effective communication, and organizational skills.
